Thank you for considering the Master of Science in Chronic Care Management (MS-CCM) at Jefferson School of Population Health.
We developed the Master of Science in Chronic Care Management to attract and educate the cadre of leaders who will move health care in a new and much-needed direction – namely, comprehensive, patient-centered management of chronic illness that extends across settings and illnesses.
It has become abundantly clear that the current methods for delivering health care are fragmented, costly, inefficient and all too often, unsafe. As we state in our program brochure, true health care reform must create the system – and the safety net – for the growing population of individuals with multiple chronic conditions.
We welcome your participation in this truly innovative and unquestionably timely graduate degree program. In an asynchronous online education format, you will participate with an online community of like-minded peers to analyze and critique models for chronic illness prevention and care delivery, consider work force demands of the future, envision seamless and coordinated care and support for chronically ill individuals and their families…and much, much more.
