Managing chronic illness is the key to true healthcare reform. No matter what its appearance – diabetes, hypertension, arthritis, cardiovascular disease, asthma, cancer or COPD (chronic obstructive pulmonary disease) – chronic disease is the greatest contributor to the cost of healthcare in the United States. Accounting for 75% of all healthcare spending, it affects an estimated 45% of the population. Among certain ethnic groups, chronic disease has reached "epidemic" proportions: African-Americans, for example, are 40% more likely to die from heart disease, and Latinos are twice as likely to die from diabetes as their white counterparts.

• Reforming health care depends more on rethinking the way we manage chronic illness than on overhauling an entire system. But reforming chronic care requires us to break out of our provider-centric and disease-focused silos to take a whole-person approach to care management
• Jefferson's Master of Science in Chronic Care Management (MS-CCM) stresses chronic illness prevention and wellness, integrates evidence-based guidelines, includes behavioral health concerns, and appreciates cultural and individual variation.
• The program prepares leaders to critically analyze, design, implement and evaluate systems to improve chronic illness prevention and care management across various health care settings and populations.
